The unveiling of the chapel stained glass window and the dedication of the chapel marks the one year anniversary of Auburn Home in Waconia. During the year, Auburn Home in Waconia, and the care provided by the staff, has inspired high praise: "It feels like home!" stated Dorothy Blaising, long time resident who made the move from the Auburn West facility.
 
According to Jerry D'Arche, a short term rehabilitation resident: "The results from my rehab have been surprising! I was very fortunate to be admitted here." Auburn Home in Waconia will be the cornerstone of senior and short term rehabilitative care in Waconia for years to come.

Auburn Home in Waconia 2008 Caregiver of the Year

Darlene Guenningsmann, Homemaker at Auburn Home in Waconia, was chosen by her peers to receive the prestigious award of Caregiver of the Year 2008 for Auburn Home in Waconia. Darlene has been employed with Moravian Care Ministries for five years. She has the love and admiration of the residents because she cares for them like members of her own family. Darlene takes the time to learn the residents' likes, dislikes, and needs and all the little nuances that make their days special. She truly is a good example of a team player. She does a wonderful job! Congratulations Darlene!

Auburn Home in Waconia is known throughout the community for providing loving, affordable care to seniors and their families. We are a skilled nursing facility with rehabilitative services including physical, occupational, and speech therapy that enable many residents to return home.
 
Licensed by the state of Minnesota and certified by both the Medicare and Medicaid programs, Auburn Home in Waconia provides many recreational, spiritual and social services in addition to 24 hour nursing care.
 
Auburn Home's mission is based on the principles of dignity, choice, care and concern. These principles are evident in the many services offered. From the moment you walk in the door, you and your loved one become part of our extended "family". Our experienced staff provides a high degree of professional and spiritual care and our goal is to keep you or your loved one as independent as possible with dignity and hope.
 
Our new care community was built on four acres just west of Ridgeview Medical Center. We have moved to a new location, but we are "moving" into a new concept of care! Our residents will live and enjoy each day in a "family home" environment.

Auburn Home in Waconia is designed to look and feel like a family home, with each wing designed as a separate household of 12 or 13 residents (37 total capacity).

  • Private dining/meeting room
  • Large chapel/multi-purpose room
  • Dedicated household for short-term, rehabilitative care
  • Beautifully landscaped patio and gazebo area.
  • Large, well-equipped Physical and Occupational Room
 
  We accept Medicare, Medical Assistance and major insurances.
 
Auburn Home in Waconia offers these specialized services:
  • Physical, occupational and speech therapy
  • Full recreation program
  • 24 hour professional, dedicated nursing staff
  • Individualized care plans
  • Wound care management
  • Pain management
  • Hospice and supportive end of life care

Putting the HOME back into the Nursing Home

Think family home, not nursing home. In addition to the design of the building, residents will have more choices and be more in charge. For example, instead of residents getting up and eating breakfast according to the staff schedule, residents will get up when they want and have what they want for breakfast. Why should someone have to change their daily routine just because they're living in a nursing home?
 
We'll be changing how we do things and how we manage things. With a flatter organizational structure and less dependence on "departmental" structures, employees will be empowered to make more decisions and be more self-directed
