At Moravian Care Ministries, we strive to provide purpose in the daily lives of each and every individual we serve. Those residing in our memory care assisted living apartments will experience physical and emotional safety that is consistently provided in a respectful and dignified manner within a structured, homelike environment.
Memory Care Core Program
The structure of the environment, along with the training and selection of the employees, creates an atmosphere of dignified acceptance for persons with memory loss issues. In addition to their own apartment, residents have a living room and den, a dining room, and a kitchen. There is a window seat, a porch, and lots of pretty walking space. It looks like home. Residents and staff eat meals together, work together, and enjoy the day together.
- Each day is filled with activity of meaning and purpose. There will be opportunities to participate in daily household routines as well as join recreation such as music, games, or religious services.
- Each resident is supported in their own level of independence. Daily self-care such as bathing and dressing are set up to maximize each resident's ability to care for themselves.
- Family is encouraged to participate at anytime, to their own level of comfort. Family is considered a member of the care team.
Basic Level of Service *( Required for every memory care resident)
- Registered Nurse interview and assessment
- Care plan development
- Medication coordination, set-up and distribution
- 24-hour home health staff on-site
- Housekeeping for individual apartment and program area
- Three main meals and snacks seven days a week
- Up to four units of personal care services per 24-hour day
For example: one-time dressing in the morning, one-time dressing in the evening, grooming, bathing, toileting, and escort to and from apartment for events/programs.
Service Levels
Service levels are determined following initial and/or admission interview and assessment with the Registered Nurse. Levels are evaluated annually and as needed for disease process progression resulting in physical and/or emotional changes which require additional support services.
- Housekeeping for individual apartment and program area

- Up to four units of personal care services per 24-hour day
